52. AMENDMENT TO CONTRACT WITH WELLNESS TO GO FOR WELL WOMAN HEALTHCHECK SERVICES Approve Amendment No. 1 to the Contract between Wellness to Go (WTG) and Maricopa County by and through its Department of Public Health (MCDPH) to provide Well Woman Healthcheck (WWHC) services to uninsured and underinsured women and whose income falls at or below 250% of the Federal Poverty Level Guidelines. This contract amendment, under 1.0 Contract Term, 1.1, is extending the end date to December 31, 2013. This contract was procured under Section MC1-328 of the Maricopa County Procurement Code, Competitive Sealed Proposals and was a successful respondent to a Request for Proposals issued by the Department of Public Health (PH RFP 10-033) on July 1, 2010. Also, this contract is funded through an Arizona Department of Health Services (ADHS) grant, WWHC (ADHS13-029502) and will not affect the county general fund. Reimbursement to the contract is on a per enrollee basis at a rate established by ADHS. (C-86-10-177-3-01)
