34. CONTRACT WITH BANNER HEALTH D/B/A BANNER HEALTH GOOD SAMARITAN MEDICAL CENTER FOR PUBLIC INFORMATION LINE Approve a Sole Source Contract for services between Banner Health d/b/a Banner Good Samaritan Medical Center (BGSMC) and Maricopa County through its Department of Public Health, Office of Preparedness and Response (OPR) formally to provide a Public Information Line (PIL). BGSMC will provide a PIL hotline that will operate 24-hours a day, seven days a week. The contract term is from July 1, 2010 through June 30, 2011 for the budget amount is not-to-exceed $134,000. Maricopa County has the right to extend this contract for additional periods, not-to-exceed a total term of four years. Materials Management has advertised this award and no other vendor has come forward to provide these services. (C-86-11-001-3-00)

Rescind action was due to the organization name change. All other terms and conditions of the original contract remain in full force and effect. (C-86-11-001-3-06)
