78. AMENDMENT TO PROVIDER AGREEMENT WITH CARE1ST HEALTH PLAN ARIZONA, INC FOR CLIENT HEALTH CARE Approve retroactive Amendment to Provider Agreement between Care1st Healthplan Arizona, Inc. (CHA) and Maricopa County through its Department of Public Health (MCDPH), Healthcare for the Homeless (HCH) for covered health care services to plan members. The amendment includes changes in the Federally Qualified Health Centers (FQHCs) reimbursement methodology. Also included is an updated Reimbursement Attachment A. The retroactive is due to processing time. This amendment is effective April 1, 2015 and will include the following changes as follows: ARTICLE II PLAN RESPONSIBILITIES, Section 2.3 is replaced; ARTICLE III PROVIDER RESPONSIBILITIES, Section 3.9 is replaced; ARTICLE III PROVIDER RESPONSIBILITIES, Section 3.24 is added; ARTICLE V UTILIZATION AND QUALITY MANAGEMENT PROGRAM, Section 5.4. is added; ARTICLE VI REGULATORY COMPLIANCE, Section 6.2 is replaced; ARTICLE VI REGULATORY COMPLIANCE, Section 6.5 is replaced; ARTICLE VI REGULATORY COMPLIANCE, Section 6.8 through 6.11 are replaced; ARTICLE VI REGULATORY COMPLIANCE, Section 6.14 is added; ARTICLE X LIABILITY, INDEMNITY AND INSURANCE, Section 10.3 is replaced; ARTICLE XV GENERAL PROVISIONS, Section 15.1 is replaced; ATTACHMENT A PROVIDER SERVICES AND COMPENSATION is replaced in its entirety; ATTACHMENT B ARIZONA HEALTH CARE COST CONTAINMENT SYSTEM (AHCCCS) Minimum Subcontract Provision and all references to ATTACHMENT B AHCCCS Minimum Subcontract Provision are removed from the agreement. This agreement allows HCH to bill CHA, an AHCCCS health plan, for reimbursable client services provided at the HCH Clinic. This allows HCH, a federally grant funded program, to generate additional revenue for the program. (C-86-11-009-3-01)
