IGA WITH MARICOPA COUNTY COMMUNITY COLLEGE DISTRICT FOR SCHOOL-BASED TOBACCO USE PREVENTION AND EDUCATION SERVICES Approve an Intergovernmental Agreement (IGA) between Maricopa County Community College District and Maricopa County by and through its Department of Public Health to provide school-based tobacco use prevention and education services for the Maricopa County Department of Public Health. Funding is for a not-to-exceed amount of $16,000 for budget period beginning July 1, 2010 through May 1, 2011. AMENDMENT TO IGA WITH MARICOPA COUNTY COMMUNITY COLLEGE DISTRICT FOR SCHOOL-BASED TOBACCO USE PREVENTION AND EDUCATION SERVICES Approve Amendment No. 1 to the Intergovernmental Agreement (IGA) between Maricopa County Community College District and Maricopa County by and through its Department of Public Health to provide school-based tobacco use prevention and education services for the Maricopa County Department of Public Health. Amendment No. 1 revises Section V, Attachment A. The contract term begins July 1, 2010 through May 1, 2011. All other terms and conditions remain in full force and effect. AMENDMENT TO IGA WITH MARICOPA COUNTY COMMUNITY COLLEGE DISTRICT FOR SCHOOL-BASED TOBACCO USE PREVENTION AND EDUCATION SERVICES Approve Amendment No. 2 to the Intergovernmental Agreement (IGA) between Maricopa County Community College District and Maricopa County by and through its Department of Public Health to provide school-based tobacco use prevention and education services for the Maricopa County Department of Public Health, which increases funding by $16,000 for budget period July 1, 2011 through May 1, 2012, for a new aggregate not-to-exceed contract total of $32,000. The contract term begins July 1, 2010 through May 1, 2012. All other terms and conditions remain in full force and effect. Funding for this agreement is provided by the Tobacco Education/Prevention Grant from ADHS and will not effect the County general fund. (C-86-11-025-3-02)

AMENDMENT TO IGA WITH MARICOPA COUNTY COMMUNITY COLLEGE DISTRICT FOR SCHOOL-BASED TOBACCO USE PREVENTION AND EDUCATION SERVICES Approve Amendment No. 3 to the Intergovernmental Agreement (IGA) between Maricopa County Community College District and Maricopa County by and through its Department of Public Health to provide school-based tobacco use prevention and education services for the Maricopa County Department of Public Health, which increases funding by $16,000 for budget period July 1, 2012 through May 1, 2013, for a new aggregate not-to-exceed contract total of $48,000. The expiration date is changed to May 1, 2013; this amendment also replaces Section 1, 22. Equal Employment Opportunity, Section III, 2. Service Goals, Revises Section III, 4. Other Requirements, A, List of Participating Schools is Attachment A. This Amendment is effective upon execution by both parties. All other terms and conditions of the original Agreement that have not been amended by this or previous amendments shall remain in full force and effect. Funding for this agreement is provided by the Tobacco Education/Prevention Grant from ADHS and will not affect the County general fund. (C-86-11-025-3-03)
