46. FACILITY USE REVOCABLE LICENSE AGREEMENT WITH PHOENIX DAY HEALTHLINKS FOR OUTREACH SERVICE AT THUNDERBIRD WIC CLINC Approve full service Facilities Use Revocable License Agreement with Phoenix Day Healthlinks (PDH) for the user of clinic space to provide Arizona Health Care Cost Containment System (AHCCCS) outreach services to uninsured Women, Infant and Children (WIC) eligible families and pregnant women, at the Thunderbird WIC clinic located at 5422 W. Thunderbird Road, Glendale 85306. The term of the Agreement is from December 1, 2010 through September 30, 2015. Licensee agrees to provide the services at no cost to the County and the recipients. County agrees to provide access without monetary remuneration. (C-86-11-033-L-00)
