66. DONATION FROM DELTA DENTAL OF ARIZONA FOUNDATION OF DENTAL SUPPLIES TO OFFICE OF ORAL HEALTH PROGRAM Accept the unsolicited donation of dental supplies from Delta Dental of Arizona Foundation (DDAF) to Maricopa County by and through its Department of Public Health, Office of Oral Health Program (MCOOH). The dental supplies are to be used as incentive gift bags to increase the return of signed consent forms by children participating in the MCOOH dental sealant program. DDAF will provide a maximum of 5,300 throughout 2011 calendar year. The total value of the dental supplies is $4,558.00. Maricopa County’s Department of Public Health’s indirect rate for FY2011 is 17%. This unsolicited in-kind donation does not allow for indirect reimbursement; therefore, indirect expenses estimated at $662 are unrecoverable. (C-86-11-042-D-00)
