33. FACILITY USE REVOCABLE LICENSE AGREEMENT WITH PHOENIX CRISIS NURSERY FOR OUTREACH SERVICE AT DOWNTOWN WIC CLINC Approve full service Facilities Use Revocable License Agreement (P50150) with Phoenix Crisis Nursery (PCH) for the use of clinic space to perform outreach services to uninsured Women, Infant and Children (WIC) eligible families and pregnant women in applying for FACES of Crisis Nursery Early Head Start Program, at the Downtown WIC clinic located at 1645 East Roosevelt Street, Phoenix, AZ 85006. The term of the agreement is from January 1, 2011 through September 30, 2014. Licensee agrees to provide the services at no cost to the County and the recipients. 52. AMENDMENT TO FACILITY USE REVOCABLE LICENSE AGREEMENT WITH THE CRISIS NURSERY, INC. Approve, and authorize the Chairman to execute amendment No. 1 to Facility Use Revocable License Agreement P-50150 with The Crisis Nursery, Inc., (TCN), an Arizona non-profit charitable corporation, for non-exclusive use of space at 1645 East Roosevelt Street, Phoenix, AZ, Maricopa County Public Health, Women, Infant and Children (WIC) clinic. The Agreement will allow TCN to perform outreach services to uninsured Women, Infants and Children (WIC) eligible families and pregnant women applying for Families and Children Experiencing Success (FACES) of Crisis Nursery Early Head Start Program The term will commence October 1, 2014 and expire September 30, 2017, unless terminated earlier as provided in the Agreement. The space will consist of the lobby area of the facility. This FURL allows TCN use of the clinic (lobby) space at the WIC facility without remuneration. There is no cost to the County or to presenting clientele. Either Party may terminate the Agreement with a 30-day notice to the other party. (C-86-11-044-1-01)

72. TERMINATE FACILITY USE REVOCABLE LICENSE AGREEMENT WITH CHILD CRISIS ARIZONA, INC. FOR USE OF OFFICE SPACE Approve and authorize the termination of License No. P-50150 with Child Crisis Arizona, Inc., as successor in interest to The Crisis Nursery, Inc., for use of the Public Health WIC facility at 1645 E Roosevelt Street, Phoenix, AZ, and authorize Real Estate Services staff to give formal notice to the User of the license termination. License P-50150 allows the County the option to terminate the agreement, without penalty, upon 30-day written notice to the User. The termination will be effective on or about August 20, 2016.
