67. GRANT WITH DELTA DENTAL OF ARIZONA FOUNDATION FOR ORAL HEALTH PROGRAM Approve a Grant from Delta Dental of Arizona Foundation (DDAF) to Maricopa County by and through its Department of Public Health, Office of Oral Health Program (MCOOH) to provide dental sealant program. This grant provides funding for a total not-to-exceed amount of $148,108 for the term of January 1, 2011 through June 30, 2012. The first payment for the grant will be paid upon receipt of signed grant agreement in the amount of $70,000. The Department of Public Health indirect rate for FY 2010-11 is 17%. Indirect costs are estimated at $18,461 and are fully recoverable. The total sum of $21,054 are sub-recipient costs and are not subject to indirect cost reimbursement. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. (C-86-11-046-3-00)
