IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR WOMEN AND CHILDREN COMMUNITY HEALTH GRANT Approve Intergovernmental Agreement (IGA) (H161084) from the Arizona Department of Health Services (ADHS) to Maricopa County through its Department of Public Health (MCDPH) for the Women and Children Community Health Group. The funding for this IGA is in the amount of $300,000 for budget period January 1, 2011 through December 31, 2011. The contract term is January 1, 2011 through December 31, 2015. The Department of Public Health indirect rate for FY 2010-11 is 17%. The grant indirect costs are estimated at $43,590 all of which are fully recoverable. Funding for this grant is provided by a grant from ADHS and does not affect the County general fund. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR WOMEN AND CHILDREN COMMUNITY HEALTH GRANT Approve Amendment No. 1 to the Intergovernmental Agreement (IGA) (H161084) between the Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) for the Women and Children Community Health Group. This Amends the Terms and Conditions of Provision Six (6) which changes the Contract number from HG161084 to ADHS12-010921. Also, the Amendment adds and deletes to the Terms and Conditions of the Agreement as stated within the Amendment. The contract term is January 1, 2011 through December 31, 2015. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR COMMUNITY HEALTH GRANT Approve Amendment No. 2 to the Intergovernmental Agreement (IGA) Contract (ADHS12-010921) between the Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide funding for the Women and Children Community Health Grant. The funding is for a not-to-exceed amount of $300,000, for the budget period effective January 1, 2012 through December 31, 2012. The contract term is January 1, 2011 through December 31, 2015. This amendment also provides additional changes to the terms and conditions. All other provisions of the Agreement remain in full force and effect. The Department of Public Health indirect rate for FY 2011-12 is 16.7%. The grant indirect costs are estimated at $42,930, all of which are fully recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR COMMUNITY HEALTH GRANT Approve Amendment No. 3 to the Intergovernmental Agreement (IGA) Contract (ADHS12-010921) between the Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide funding for the Women and Children Community Health Grant. The funding is for a not-to-exceed amount of $300,000 for the budget period effective January 1, 2013 through December 31, 2013. The contract term is January 1, 2011 through December 31, 2015. The Department of Public Health indirect rate for FY13-14 is 15.8%. The grant indirect costs are estimated at $40,932, all of which are fully recoverable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. The funding is for the not-to-exceed amount of $300,000 for the budget period January 1, 2014 through December 31, 2014. The Agreement term is January 1, 2011 through December 31, 2015. The Department of Public Health’s indirect rate for FY2013-14 is 15%. The grant indirect costs are estimated at $39,130 of which is fully recoverable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. This Amendment revises the current price sheet Section 1.1 Personnel increased by $1,477.68 for a total of $181,624.68 due to salary increase by 2.5%, and ERE increased by $2,191.73 for a total of $68,461.73. The budget total remains the same at $300,000.00 The Department of Public Health’s indirect rate for FY2014-15 is 14.8%. The grant indirect costs are estimated at $38,676 of which is fully recoverable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation.
