36. CONTRACT WITH WILLDAN FOR PREPAREDNESS EXERCISE DESIGN, DEVELOPMENT AND EXECUTION Approve the Contract Pursuant to PH-RFP 11-002, between Maricopa County through the Department of Public Health, and Willdan for the design, development and execution of multiple preparedness exercises. 40. AMENDMENT TO CONTRACT WITH WILLDAN FOR PREPAREDNESS EXERCISE DESIGN, DEVELOPMENT AND EXECUTION Approve Amendment No. 1 to the Contract Pursuant to PH-RFP 11-002, between Maricopa County through the Department of Public Health, and Willdan for the design, development and execution of multiple preparedness exercises. Amendment No.1 adds an additional Scope of Work and increases funding by $100,000, for the budget period January 26, 2012 through January 25, 2013. The new contract total is in the not-to-exceed amount of $220,000 for the Contract period January 26, 2011 through January 25, 2013. All other terms and conditions remain in full effect. AMENDMENT TO CONTRACT WITH WILLDAN FOR PREPAREDNESS EXERCISE DESIGN, DEVELOPMENT AND EXECUTION Approve Amendment No. 3 to the Contract Pursuant to PH-RFP 11-002, between Maricopa County through the Department of Public Health, and Willdan to extend the Contract term ending on April 30, 2013, therefore, the contract period is January 26, 2011 through April 30, 2013. All other terms and conditions of the original contract and any subsequent amendments shall remain in full force and effect. (C-86-11-049-3-03)
