FACILITY USE REVOCABLE LICENSE AGREEMENT WITH ARIZONA BOARD OF REGENTS ARIZONA STATE UNIVERSITY FOR HEALTH EDUCATIONAL OUTREACH AT SOUTH PHOENIX WIC CLINC Approve full service Facilities Use Revocable License Agreement (P50159) by and between the Arizona Board of Regents for and on behalf the Arizona State University, College of Nursing and Health Innovation Program (CNHIP) for the use of clinic space to perform health educational outreach to uninsured Women, Infant and Children (WIC) eligible families and pregnant women at the South Phoenix WIC clinic located at 438 East Southern Avenue, Phoenix, AZ 85040. The term of the agreement is from June 1, 2011 through September 30, 2012. Licensee agrees to provide the services at no cost to the County and the recipients. AMENDMENT TO FACILITIES USE/REVOCABLE LICENSE AGREEMENT WITH ARIZONA BOARD OF REGENTS ARIZONA STATE UNIVERSITY FOR HEALTH EDUCATION OUTREACH AT SOUTH PHOENIX WIC CLINIC Approve and authorize the Chairman to execute, Amendment No. 1 to License No. P50159 with Arizona Board of Regents Arizona State University for use of space at the Public Health/WIC facility to perform health educational outreach to uninsured Women, Infant and Children (WIC) located at 438 E. Southern Ave., Phoenix, AZ, 85040. Amendment No. 1 will extend the current License Agreement one year to commence October 1, 2012 and to terminate on September 30, 2013. The Licensee agrees to provide the services at no cost to the County and the recipients. County agrees to provide access without monetary remuneration. (C-86-11-060-1-01)
