AMENDMENT TO CONTRACT WITH DELTA DENTAL OF ARIZONA FOUNDATION FOR ORAL HEALTH SERVICES Approved Amendment No. 1 to a pre-designated fixed price Contract between Delta Dental of Arizona Foundation (DDFA) and Maricopa County by and through its Department of Public Health Service (MCDPH), Office of Oral Health to provide oral health services including dental education, screening and fluoride varnish. The contract amount is increased by not-to-exceed $44,000, for an aggregate total not-to-exceed $65,000. The budget period is July 1, 2011 through June 30, 2012. Also, the expiration date has been extended to June 30, 2012. All other terms and conditions of the original contract shall remain in full force and effect. CONTRACT WITH DELTA DENTAL OF ARIZONA FOUNDATION FOR ORAL HEALTH SERVICES Approve Amendment No. 2 to a pre-designated fixed price contract between Delta Dental of Arizona Foundation (DDFA) and Maricopa County by and through its Department of Public Health Service (MCDPH), Office of Oral Health to provide oral health services including dental education, screening, and fluoride varnish. The Contract amount is increased by not-to-exceed $44,000, for an aggregate total not-to-exceed $109,000.00. The budget period is July 1, 2012 through June 30, 2013. All other terms and conditions of the original contract and any amendments shall remain in full force and effect. Fees for Oral Health Services are paid through a grant fund from Arizona Early Childhood Development and Health Board (AECDHB), First Things First (FTF). The grant award for Oral Health Services with Delta Dental of Arizona Foundation was approved by the Board on April 17, 2011 (C-86-11-073-3-00) and Arizona Early Childhood Development and Health Board was approved by the Board on October 6, 2010 (C-86-11-014-3-00). Procurement authority is Prior Designation – MC1-102C. Funding for this contract is provided by a grant and will not impact the County general fund budget. (C-86-11-073-3-02)

The contract amount is increased by not-to-exceed $44,000, for an aggregate total not-to-exceed $153,000. The budget period is July 1, 2013 through June 30, 2014. Also, the expiration date is extended to June 30, 2014. All other terms and conditions of the original contract and any amendments shall remain in full force and effect. (C-86-11-073-3-03)

IGA WITH MARICOPA COUNTY SPECIAL HEALTH CARE DISTRICT DBA MARICOPA INTEGRATED HEALTH CARE SYSTEM FOR HEALTHCARE FOR THE HOMELESS ANCILLARY AND SPECIALTY SERVICES Approve IGA between Maricopa County Special Healthcare District d.b.a. Maricopa Integrated Health System (MIHS) and Maricopa County through and by its Department of Public Health (MCDPH), Healthcare for the Homeless (HCH program to provide Ancillary and Specialty Services for homeless clients. This IGA provides funding in an amount not-to-exceed $100,000 per year for the Budget period July 1, 2016 through June 30, 2021. The term of the Agreement is July 1, 2016 through June 30, 2021. This IGA is a subcontract to the HCH Notice of Award and Contract (C-86-11-073-3-38) which is funded by HRSA and approved on 11-19-2014. This will not affect County funds. (C-86-16-103-3-00)
