30. GRANT CONTRACT FROM PHOENIX AFFILIATE OF SUSAN G. KOMEN BREAST CANCER FOUNDATION INC FOR THE WELL WOMAN HEALTHCHECK PROGRAM Approve Grant Contract from the Phoenix Affiliate of the Susan G. Komen Breast Cancer Foundation, Inc. d/b/a The Phoenix Affiliate of Susan G. Komen for the Cure to Maricopa County through its Department of Public Health to provide the Well Woman Healthcheck Program Cycle of Care Project. Grant funding is in the amount of $150,000 for budget term beginning April 1, 2011 through March 31, 2012. The Department of Public Health’s indirect rate is 17% for FY 2011-12. Total indirect costs are estimated at $25,500, all of which is unrecoverable. The total sum of the grant consists of sub-recipient pass-through funds not subject to indirect costs. Funding for this grant is provided by the Susan G. Komen Breast Cancer Foundation, Inc. d/b/a the Phoenix Affiliate of the Susan G. Komen for the Cure and will not affect the County general fund.
