40. FACILITY USE REVOCABLE LICENSE AGREEMENT WITH CATHOLIC HEALTHCARE WEST D/B/A CHANDER REGIONAL MEDICAL CENTER FOR ORAL HEALTH, HEARING AND VISION SERVICES AT GUADALUPE WIC CLINIC Approve full service Facilities Use/Revocable License Agreement with Catholic Healthcare West, a California non-profit public benefit Corporation d/b/a Chandler Regional Medical Center, Licensee, for the user of clinic space to perform oral health, hearing and vision services to underserved WIC eligible children ages 0 to five, their families and pregnant women, at the Guadalupe WIC clinic located at 9206 S. Avenida del Yaqui, Guadalupe, AZ 85283. The term of the Agreement is from May 1, 2011 through April 30, 2012. License agrees to provide the services at no cost to the County and the recipients. 60. AMENDMENT TO FACILITY USE REVOCABLE LICENSE AGREEMENT WITH DIGNITY HEALTH FOR ORAL HEALTH, EDUCATION, HEARING AND VISION SCREENINGS AT GUADALUPE WIC Approve Amendment No. 2 to full service Facilities Use/Revocable License Agreement (FURL) (P50171) between the Dignity Health, a California non-profit public benefit corporation d/b/a Chandler Region Medical Center (CRMC), Licensee, and Maricopa County by and through its Department of Public Health, Women, Infant and Children (WIC) Agency, for the user of clinic space to perform Oral Health, Hearing and Vision screening; Oral Health Education to underserved WIC eligible children ages zero (0) to five (5), their families and pregnant women at the WIC clinic located at 9206 S Avenida del Yaqui, Guadalupe, AZ 85283. The term of the FURL is from May 1, 2013 to April 30, 2014. This amendment also removes and replaces Exhibit A. All other terms and conditions of the original agreement and its amendments shall remain in full force and effect. The Facility Review Committee has reviewed this Facility Use/Revocable Agreement at the February 2013 meeting. This Use Permit Agreement also allows for CRMC the use of clinic space at the Guadalupe WIC facility as scheduled on Exhibit “A”. Licensee agrees to provide the services at no cost to the County and the recipients. County agrees to provide access without monetary remuneration (C-86-11-079-M-02)

45. AMENDMENT TO FACILITY USE REVOCABLE LICENSE AGREEMENT WITH DIGNITY HEALTH Approve and execute Amendment No. 3, to the full service Facilities Use/Revocable License Agreement No. P50171 with Dignity Health, an Arizona non-profit corporation dba Chandler Regional Medical Center, “User”, for the use of clinic space to administer fluoride varnish treatments and perform oral health screenings, hearing and vision services to uninsured children ages 0-5 years, their families and pregnant women at the Guadalupe WIC clinic located at 9026 S. Avenida del Yaqui, Guadalupe, AZ 85283. The term of the Agreement is from May 1, 2014 through October 31, 2014. Licensee agrees to provide the services at no cost to the County and the recipients. County agrees to provide access and promotion assistance without monetary remuneration. This Facility Use/Revocable License Agreement allows Dignity Health the use of clinic space at the Guadalupe WIC facility as scheduled at Exhibit “A”. Oral health, hearing and vision services to be provided to non-insured recipients at no cost or remuneration to the County. This general type of Real Estate action was endorsed by the Facilities Review Committee during August of 2013. (C-86-11-079-M-03)
