56. LEASE AGREEMENT L-7431 WITH PATRICK SHIELDS AND GABRIELLE SHIELDS Approve and authorize the Chairman to execute Lease Agreement L-7431 between Patrick Shields and Gabriella Shields, husband and wife as community property with right of survivorship, Lessor, and Maricopa County (MC) for 4,699 square feet (sf) of office space at 8828 North Central Avenue, Phoenix, Arizona. The term of this lease shall be for a period of five years commencing after approval and execution of the lease agreement. The base rent plus rental tax: Year 1, $75,371.96 ($16.04/sf), Year 2, $77.721.46 ($16.54/sf), Year 3, $80,070.96 ($17.04/sf), Year 4, $82,420.46 ($17.54/sf), Year 5, $84,769.96 ($18.04/sf). The lease contains a six (6) month holdover provision. This Lease Agreement L-7431 was endorsed by the County’s Facility Review Committee during April of 2011. (C-86-11-086-1-00)

ADMINISTRATIVE CORRECTION TO AMENDMENT TO LEASE AGREEMENT WITH PATRICK SHIELDS AND GABRIELLE SHIELDS Approve an Administrative Correction to an action taken on May 25, 2011 (C-86-11-086-1-00) that approved and authorized the Chairman to execute Lease Agreement L-7431 between Patrick Shields and Gabriella Shields, husband and wife as community property with right of survivorship as Lessor, and Maricopa County for 4,699 square feet (sf) of office space at 8828 North Central Avenue, Phoenix, Arizona. The correction will add the following language: Additional costs incurred other than rent includes utilities and common area maintenance (CAM) for approximately $1,860.02 monthly. This Lease Agreement L-7431 was endorsed by the County’s Facility Review Committee during April of 2011. (C-86-11-086-1-01)

AMENDMENT TO LEASE AGREEMENT L-7431 WITH PATRICK SHIELDS AND GABRIELLE SHIELDS Approve and execute Amendment No. 1 to Lease No. L-7431 with Patrick Shields and Gabriella Shields, husband and wife with right of survivorship, Lessor for 4,699 square feet of leased space located at 8828 N. Central Ave, Phoenix, AZ. This amendment will add additional language at paragraph 1.3 "Tenant Improvements” which will note the additional tenant improvements which Lessor provided and facilitate the one-time cost paid by Lessee not to exceed $14,000.00. All other terms and conditions of the current Lease Agreement remain in full force and effect. This action was endorsed by the County Facility Review Committee during the August 2011 meeting. 50. AMENDMENT TO LEASE AGREEMENT WITH PGS PROPERTY MANAGERS, LLC FOR OFFICE SPACE Approve and authorize the Chairman to execute Amendment No. 2 to Lease Agreement L-7431 with PGS Property Managers, LLC, as successors in interest to Patrick Shields and Gabrielle Shields, husband and wife, as community property with right of survivorship, Lessor, for 4,699 square feet of office space at 8828 N. Central Avenue, Phoenix, AZ. This amendment will extend the term of the lease 5 years to expire July 21, 2021. The rate will decrease from $18.04 to $12.00 per square foot plus rental tax with annual increases of $0.50 per square foot. The amendment also expands the Use of Premises to include associated agency health providers and strategic partners. (C-86-11-086-1-03)
