A-7. FACILITY USE REVOCABLE LICENSE AGREEMENT WITH EMPOWERMENT SYSTEMS INC FOR KIDS HEALTH LINKS SERVICES AT GREENFIELD WIC Approve full service Facilities Use/Revocable License Agreement between Empowerment Systems, Inc., Licensee, and Maricopa County by and through its Department of Public Health, Women, Infant and Children (WIC) Agency, for the user of clinic space to perform Kids Health Links Services to underserved WIC eligible children ages zero (0) to five (5), their families and pregnant women at the WIC clinic located at 4419 East Main, Mesa, AZ 85205. The term of the Amendment starts from June 1, 2011 and ends May 30, 2012. Licensee agrees to provide the services at no cost to the County and the recipients. County agrees to provide access without monetary remuneration. This Use Permit Agreement allows for CRMC the use of clinic space at the Greenfield WIC facility as scheduled on Exhibit “A”. The Facility Review Committee has reviewed this Facility Use/Revocable Agreement. (C-86-11-092-M-00)

69. AMENDMENT TO FACILITY USE REVOCABLE LICENSE AGREEMENT NO. P50170 WITH EMPOWERMENT SYSTEMS, INC. Approve Amendment No. 2 to Facility Use Revocable License Agreement No. P50170 between Empowerment Systems, Inc. (User) and Maricopa County, by and through the Department of Public Health, (County) for the use of leased space at the WIC facility located at 4419 East Main, Mesa, AZ 85205. This amendment will transfer the services to a more accommodating WIC facility located at 430 N. Dobson, Suites 111 & 112, Mesa, AZ. All other terms and conditions of the original Facility Use Revocable License Agreement subsequent amendment will remain the same and in full force and effect. (C-86-11-092-M-02)

AMENDMENT TO FACILITY USE REVOCABLE LICENSE AGREEMENT WITH EMPOWERMENT SYSTEMS FOR KIDS HEALTH LINKS SERVICES AT WIC CLINIC Approve Amendment No. 3 to full service Facilities Use/Revocable License Agreement No. P50170 between Empowerment Systems, Inc., Licensee, and Maricopa County by and through its Department of Public Health, Women, Infant and Children (WIC) Agency, for the user of clinic space to perform Kids Health Links Services to underserved WIC eligible children ages zero (0) to five (5), their families and pregnant women at the WIC clinic located at 430 N Dobson, Suites 111 & 112, Mesa, AZ 85205. The term of the Amendment is extended one (1) year commencing June 1, 2013 to May 31, 2014. Licensee agrees to provide the services at no cost to the County and the recipients. County agrees to provide access without monetary remuneration. All other terms and conditions of the original agreement shall remain in full force and effect. This Use Permit Agreement allows for Empowerment Systems, Inc, the use of clinic space at the Mesa WIC facility as scheduled on Exhibit “A”.
