LEASE AGREEMENT WITH PUBLIC STORAGE FOR SUPPLY STORAGE SPACE Approve and execute Lease/Rental Agreement No. L-7434 with Public Storage located at 4717 N. 43RD Ave. Phoenix, AZ for a 20X10 square foot private storage space. The monthly rental rate is $128.00 plus applicable tax and a $22.00 one-time new account fee. The Lease Agreement is month-to-month with a commencement date of May 19, 2011. Public Health may terminate this agreement with a seven (7) day notice to the Landlord. (C-86-11-097-1-00)
