94. IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR CARDIOVASCULAR DISEASE PREVENTION Approve Intergovernmental Agreement (IGA) (No.ADHS11-007632) between the Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health for the Cardiovascular Disease Prevention Program. Total funding provided is in the not-to-exceed amount of $100,000, for budget period June 29, 2011 to June 28, 2012. The term of this IGA is June 29, 2011 through June 28, 2016. The Department of Public Health FY12 indirect rate is 16.7%. Indirect costs are estimated at $14,310 and are fully recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funds for this IGA are provided by ADHS and do not affect the County’s general fund.
