CONTRACT WITH DISTRICT MEDICAL GROUP (DMG), AN ARIZONA COPRPORATION FOR THE PURCHASE OF ANCILLARY MEDICAL SERVICES Approve a Contract between Maricopa County, a political subdivision of the State of Arizona, through its Department of Public Health and District Medical Group (DMG), an Arizona corporation, for the purchase of Ancillary Medical services. The term of the contract begins August 1, 2011 and ends July 31, 2012. The Contract is for a term of one year with the option of renewal for additional terms up to five years. AMENDMENT TO CONTRACT WITH DISTRICT MEDICAL GROUP, AN ARIZONA COPRPORATION FOR THE PURCHASE OF ANCILLARY MEDICAL SERVICES Approve Amendment No. 1 to Request for Proposal (RFP) Contract (PH RFP 11-011) Contract between District Medical Group (DMG), an Arizona corporation and Maricopa County through its Department of Public Health (MCDPH) for the purchase of Ancillary Medical services. This Amendment will extend the RFP contract period for an additional year beginning August 1, 2012 ending July 31, 2013. All other terms and conditions of the original contract shall remain in full force and effect. DMG is a successful respondent to a Request for Proposal (PH RFP 11-011) issued by Maricopa County Department of Public Health (MCDPH) procured under Section MC1-330 of the Maricopa County Procurement Code Competitive Sealed Proposals.
