95. AGREEMENT FOR HEALTHY START SERVICES WITH VHS ACQUISITION CORPORATION D/B/A MARYVALE HOSPITAL Approve the Agreement to Healthy Start Services between VHS Acquisition Corporation D/B/A Maryvale Hospital and Maricopa County through its Department; Office of Family Health (OFH), to collaborate with the Maryvale to provide Health Start Services to eligible clients. The term of this non-financial agreement shall begin upon execution by both parties and shall be in effect until terminated by either party. (C-86-12-006-3-00)
