CONTRACT WITH HEALTH RESOURCES IN ACTION FOR THE PURCHASE OF PUBLIC HEALTH POLICY ANALYSIS SERVICES Approve a Contract between Maricopa County, a political subdivision of the State of Arizona, through its Department of Public Health and Health Resources in Action for the purchase of Public Health Policy Analysis services. The fund amount is $120,000 and the term of the Contract begins August 1, 2011 and ends July 31, 2012. AMENDMENT TO CONTRACT WITH HEALTH RESOURCES IN ACTION FOR THE PURCHASE OF PUBLIC HEALTH POLICY ANALYSIS SERVICES Approve Amendment No. 3 to Request for Proposal Contract (PH RFP 11-016) between Health Resources in Action (HRIA) and Maricopa County by and through its Department of Public Health, Policy to provide Public Health Policy Analysis Services. The amendment removes and replaces Section 6.25.1 Pricing, Exhibit A-1, increasing the budget amount by $47,220, bringing the total not to exceed amount from $81,140 to $128,360 for the project period February 1, 2013 to June 30, 2013 and adding Section 6.26.2, Exhibit B-1, Scope of Work. The Contract is for a term of one (1) year with the option of renewal for additional terms up to five (5) years. This contract was procured under Section MC1-330 of the Maricopa County Procurement Code, Competitive Sealed Proposals. HRIA was a successful respondent to a Request for Proposals issued by the Department of Public Health on August 1, 2011. (C-86-12-010-3-03)
