57. PURCHASE ORDER WITH ARIZONA DEPARTMENT OF HEALTH SERVICE FOR TUBERCULOSIS CONTROL Approve Purchase Order (HG854521:1) from Arizona Department of Health Services (ADHS) to Maricopa County through its Department of Public Health for grant funding for the Intergovernmental Agreement (IGA) HG854521 for the Tuberculosis Control Program (TB). The purchase order is for the budget period starting July 1, 2011 through June 30, 2012, for the purchase order total of $281,650 however this purchase order covers the first quarter of $70,412.50. The term of this IGA is July 1, 2008 through June 30, 2013. The Department of Public Health indirect rate for FY 2011-2012 is 16.7%. Indirect Cost are estimated at $40,305, all of which is fully recoverable. Funds for this IGA are provided by a grant from ADHS and does not affect the County’s general fund. (C-86-12-018-3-00)
