57. PROVIDER AGREEMENT WITH PHOENIX HEALTH PLAN FOR THE PROVISION OF HEALTH CARE SERVICES Approve the Provider Agreement between Phoenix Health Plan, LLC (PHP) and Maricopa County, dba, Maricopa County Department of Health Services, dba, Public Health Clinical (PHC). The term of this Agreement shall begin upon Board of Supervisor approval for a period of one year and with automatic one-year renewals with a 90 day termination clause. Estimated revenue amount is $30,000 per year and may change based on the number of clients actually serviced. There is no cap. This Agreement allows PHC to bill Phoenix Health Plan, an Arizona Health Care Cost Containment System (AHCCCS) health plan, for reimbursable client services provided at the PHC clinic. This allows PHC, a federally grant funded program, to generate additional revenue for the program. The overall fee fund budget will be adjusted as necessary to accommodate increase in revenue through a future reconciliation. (C-86-12-026-3-00)
