39. CONTRACT WITH CITY OF PHOENIX HUMAN SERVICES DEPARTMENT HEAD START PROGRAM FOR TB SCREENING Approve a Contract Award between City of Phoenix (COP), Human Services Department (HSD), Head Start Program (HSP) and Maricopa County through its Department of Public Health (MCDPH), Clinical Service, Tuberculosis (TB) (Contract No. 131562) to provide Tuberculosis Screenings to Head Start referrals. The COP HSP referrals will be for parents or children who need diagnostic services for latent or active TB infection including TB Skin tests and x-rays. The contract term is July 1, 2011 and ends June 30, 2012 with option to renew for two additional one-year periods. The fund is not- to -exceed $49,999 to be expended during the entire contract term. This contract amount is included in fee fund FY 2011-2012 recommended budget. (C-86-12-034-3-00)

AMENDMENT TO CONTRACT WITH CITY OF PHOENIX HUMAN SERVICES DEPARTMENT HEAD START PROGRAM FOR TB SCREENING Approve Amendment No. 1 between City of Phoenix (COP), Human Services Department (HSD), Head Start Program (HSP) and Maricopa County through its Department of Public Health (MCDPH), Clinical Service, Tuberculosis (TB) (Contract No. 131562-001) to provide Tuberculosis Screenings to Head Start referrals. The CP HSP referrals will be for parents or children who need diagnostic services for latent or active TB infection including TB Skin tests and x-rays. The Amendment revises the language related to indemnification and insurance requirement. All other term and conditions of the original contract remain in full force and effect. (C-86-12-034-3-01)
