58. FACILITY USE REVOCABLE LICENSE AGREEMENT WITH KEOGH HEALTHCARE CONNECTION TO PROVIDE OUTREACH SERVICES FOR WIC CLIENTS Approve full service Facilities Use/Revocable License Agreement (P50183) between, Licensee, Keogh Healthcare Connection (KHC) and Maricopa County by and through its Department of Public Health, Women, Infant and Children (WIC) Agency, for the user of clinic space to provide assistance with enrollment into Department of Economic Services (DES), Arizona Health Care Cost Containment System (AHCCCS), Food Stamps and Cash Assistance services, to underserved WIC eligible children ages zero (0) to five (5), their families and pregnant women at the WIC clinic located at 1260 South 7th Avenue, Phoenix, AZ 85007. The term of this agreement starts from December 1, 2011 and ends November 30, 2012. Licensee agrees to provide the services at no cost to the County and the recipients. County agrees to provide access without monetary remuneration. All other terms and conditions of the original agreement shall remain in full force and effect. This Use Permit Agreement allows for CRMC the use of clinic space at the Seventh Avenue WIC facility as scheduled on Exhibit “A”. 42. AMENDMENT TO FACILITY USE/REVOCABLE LICENSE AGREEMENT NO. P-50183 WITH KEOGH HEALTH Approve and execute Amendment No. 2 to Facility Use/Revocable License No. P50183 with Keogh Health as user of County owned space located at 1260 South 7th Avenue, Phoenix, AZ 85007 currently being used by Public Health/Women, Infants and Children (WIC) staff serving women, infants and children. Keogh Health will provide assistance with enrollment into Department of Economic Services (DES), Arizona Health Care Cost Containment Systems (AHCCCS), Food Stamps and Cash Assistance services to underserved WIC eligible children ages zero (0) to five (5), their families and pregnant women. This Amendment will extend the existing License Agreement from December 1, 2013 to November 30, 2014 at no charge to the County or County clients. (C-86-12-042-M-02)
