A-3. CONTRACT WITH CENTRAL ARIZONA SHELTER SERVICES TO PROVIDE DENTAL SERVICES FOR THE HOMELESS Approve a Sole Source Contract between Maricopa County through its Department of Public Health, Health Care for the Homeless (HCH) and Central Arizona Shelter Services (CASS) to provide dental services to homeless individuals. The sole source contract is in compliance with the Maricopa County Procurement Code, Section MC1-344. This contract provides funds to CASS in the amount of $75,000 for the budget period November 1, 2011 to October 31, 2012 with the option of extending the contract for an additional five (5) years for an aggregate not-to-exceed total of $375,000. Funding for this contract is provided by a grant from the Health Resources and Services Administration and will not affect the County General Fund. (C-86-12-043-3-00)

46. AMENDMENT TO CONTRACT WITH CENTRAL ARIZONA SHELTER SERVICES TO PROVIDE DENTAL SERVICES FOR THE HOMELESS Approve Amendment No. 1 to Sole Source Contract between Central Arizona Shelter Services (CASS) and Maricopa County through its Department of Public Health (MCDPH), Health Care for the Homeless (HCH), to provide dental services to homeless individuals. This amendment extends the contract period from November 1, 2012 to October 31, 2013 with the option of extending the contract for not to exceed term of five (5) years. The contract term is from November 1, 2011 through October 31, 2016. All other terms and conditions of the original contract and all amendments shall remain in full force and effect. This sole source contract with CASS is in compliance with the Maricopa County Procurement Code, Section MC1-344. All supporting documents are on file. Funding for this contract is provided by a grant from the Health Resources and Services Administration and will not affect the County General Fund. (C-86-12-043-3-01)
