21. IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICE FOR HYPERTENSION SYSTEM OF CARE PROGRAM Approve Intergovernmental Agreement (IGA) (ADHS12-013061) between Arizona Department of Health Services and Maricopa County through its Department of Public Health to provide funding for the Tobacco - Hypertension System of Care program, in the not-to-exceed amount of $106,000.00, for the budget period October 1, 2011 through September 30, 2012. The term of the IGA is October 1, 2011 through September 30, 2016. All other provisions of this agreement remain unchanged. The Department of Public Health indirect rate for FY11-12 is 16.7%. Grant indirect costs are 15% of salaries and employee related expenses of $19,448. The full indirect costs are estimated at $15,169 of which $2,917 is allowable and $12,252 is not allowable. Funds for this IGA are provided by a grant from ADHS and do not affect the County’s general fund. (C-86-12-047-3-00)

AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICE FOR HYPERTENSION SYSTEM OF CARE PROGRAM Approve Amendment to Intergovernmental Agreement (IGA) (ADHS12-013061) between Arizona Department of Health Services and Maricopa County through its department of Public Health to provide funding for the Tobacco – Hypertension System of Care Program, in the not-to-exceed amount of $80,500, for the budget period July 1, 2012 through June 30, 2013. The amendment also replaces D. Task and Responsibilities, and replaces E. Deliverables, items to be submitted 3, 4, 5, and 6. The term of the IGA is October 1, 2011 through September 30, 2016. The Department of Public Health indirect rate for FY12-13 is 15.8%. Grant indirect costs are 15% of salaries and employee related expenses of $8,576. The full indirect costs are estimated at $1,358 of which $1,286 is allowable and $72 is not allowable. Funds for this IGA are provided by a grant from ADHS and do not affect the County’s general fund. (C-86-12-047-3-01)
