22. IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR CHRONIC DISEASE HEALTH POLICY Approve Intergovernmental Agreement (IGA) Contract No. (ADHS12-015902) between the Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide funding for Chronic Disease Health Policy program. The funding is for a not-to-exceed amount of $412,370, for the budget period effective February 1, 2012 through January 31, 2013. The contract term shall be effective as of the begin date February 1, 2012 and shall remain effective until the termination date January 31, 2017. The Department of Public Health indirect rate for FY11-12 is 16.7%. Grant indirect costs are 15% of salaries and employee related expenses of $87,696. The full indirect costs are estimated at $59,011 of which $13,154 is allowable and $45,857 is not allowable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funding for this agreement is provided by a Grant through ADHS by Centers for Disease Control & Prevention (CDC) Chronic Disease Prevention and Health Promotion (CDPHP) and will not affect the County general fund. IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR CHRONIC DISEASE HEALTH POLICY Approve Purchase Order (ADHS12-015902:2) to Intergovernmental Agreement (IGA) Contract No. (ADHS12-015902) between the Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide funding for Chronic Disease Health Policy program. The funding is for a not-to-exceed amount of $412,370, for the budget period effective February 1, 2013 through January 31, 2014. The contract term shall be effective as of the begin date February 1, 2012 and shall remain effective until the termination date January 31, 2017. Terms and conditions set forth in the ADHS bid, quotation, or purchase order are incorporated herein by reference and become a part of this order. The Department of Public Health indirect rate for FY12-13 is 15.8%. Grant indirect costs are 15% of salaries and employee related expenses of $105,993. The full indirect costs are estimated at $56,265 of which $15,899 is allowable and $40,366 is not allowable Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. ADMINISTRATIVE CORRECTION TO AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR CHRONIC DISEASE HEALTH POLICY Approve an Administrative Correction to Amendment No. 1 to the Intergovernmental Agreement between Maricopa County, by and through its Department of Public Health, and Arizona Department of Health Services (ADHS) for Chronic Disease Health Policy to correct the term end date from January 31, 2014 to March 31, 2014. All other terms and conditions of the original agreement are in full force and affect. No additional funds are included in this Amendment. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. The Contract term is effective February 1, 2012 through January 31, 2017. All other terms and conditions remain in full force and effect. The Department of Public Health’s indirect rate for FY14 is 15%. Grant indirect costs are 15% of salaries and employee related expenses at $91,403. The full indirect costs are estimated at $37,696 of which $13,710 is allowable and $23,986 is not allowable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. The Price Sheet is replaced and increased in the not-to-exceed amount of $75,000 for the grant extension April through June 2015. Terms and Conditions, 18. Health Insurance Portability and Accountability Act of 1996 is deleted in its entirety and replaced with page 2 of this Amendment. All other terms and conditions remain in full force and effect. The Department of Public Health’s indirect rate for FY15 is 14.8%. Grant indirect costs are 15% of salaries and employee related expenses at $22,808. The full indirect costs are estimated to increase by $9,669 of which $3,421 is allowable and $6,248 is not allowable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105.
