36. HEAPLUS COMMUNITY PARTNER AGREEMENT AMENDMENT #3 To approve Amendment #3 to the Agreement between Maricopa County and Arizona Health Care Cost Containment System (AHCCCS) for Health-e-Arizona Plus (HEAplus). Correctional Health Services (CHS) is one Community Partner Organization as part of this Agreement. The CHS Community Transitions team uses the HEAplus system to verify AHCCCS status/eligibility and complete pre-release AHCCCS applications for justice-involved individuals transitioning from jail back into the community. Amendment #3 includes additional language for termination as established by AHCCCS and all other terms remain unchanged. (C-86-12-056-3-04)
