69. FACILITIES USE/REVOCABLE LICENSE AGREEMENT WITH THE CITY OF PHOENIX HEAD START PROGRAM FOR COMMUNITY OUTREACH AT MARYVALE WIC CLINIC Approve full service Facilities Use/Revocable License Agreement (P50191) between the City of Phoenix Services Department, Early Head Start and Head Start Program (Licensee) and Maricopa County by and through its Department of Public Health, Women, Infant and Children (WIC) Agency, (Licensor), for Licensee to perform outreach to the community, and to assist families in applying for the Head Start Program to WIC eligible underserved families. The term of this Agreement is for three (3) years and shall commence on the date the Agreement is approved by the County and City. The City agrees to provide the Services at no cost to the County and Recipients. The County agrees to provide the City access at the current Maryvale WIC Facility located at 4002 North 67th Avenue, Suite 10, Phoenix, AZ. (C-86-12-061-M-00)
