73. FACILITIES USE/REVOCABLE LICENSE AGREEMENT WITH THE CITY OF PHOENIX HEAD START PROGRAM FOR COMMUNITY OUTREACH AT NORTH VALLEY WIC CLINIC Approve full service Facilities Use/Revocable License Agreement (P50195) between the City of Phoenix Services Department, Early Head Start and Head Start Program (Licensee) and Maricopa County by and through its Department of Public Health, Women, Infant and Children (WIC) Agency, (Licensor), for Licensee to perform outreach to the community, and to assist families in applying for the Head Start Program to WIC eligible underserved families. The term of this Agreement is for three (3) years and shall commence on the date the Agreement is approved by the County and City. The City agrees to provide the Services at no cost to the County and Recipients. The County agrees to provide the City access at the current North Valley WIC Facility located at 19401 North Cave Creek Road, Phoenix, AZ. (C-86-12-065-M-00)

27. AMENDMENT TO FACILITIES USE/REVOCABLE LICENSE AGREEMENT NO. P-50195 WITH CITY OF PHOENIX Approve and execute Amendment No. 1 to Facilities Use/Revocable License Agreement No. P-50195 between the City of Phoenix Human Services Department, Early Head Start and Head Start Program (Licensee) 133352 (1) and Maricopa County by and through its Department of Public Health, Women, Infant and Children (WIC) Agency, (Licensor), for Licensee to perform outreach to the community, and to assist families in applying for the Head Start Program to WIC eligible underserved families. The current term expires April 30, 2015. The term of this Agreement is for one (1) year and will automatically renew for one (1) year. The term shall commence on May 1, 2015 and expire April 30, 2017. The City agrees to provide the Services at no cost to the County and Recipients. The County agrees to provide the City access to the WIC Facility located at 16401 North Cave Creek Road, Phoenix, AZ. Either Party may terminate the Agreement with a 30-day written notice to the other. (C-86-12-065-M-01)
