26. AMENDMENT TO IGA WITH ARZONA DEPARTMENT OF HEALTH SERVICES HIV SURVEILLANCE PROGRAM Approve amendment No. 3 to the Intergovernmental Agreement (IGA) Contract No. ADHS17-141746, HIV Surveillance program between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health. The price sheet is in the not to exceed amount of $237,807 for the CY20 budget period beginning January 1, 2020 through December 31, 2020. The term of the IGA is from January 1, 2017 through December 31, 2021. The Intergovernmental Agreement referenced is amended as follows: 1. Pursuant to Terms and Conditions, Provision Six (6), Contract Changes, Item 6.1, Amendments, Purchase Orders and Change Orders, the Contract is hereby amended as follows: 1.1. The Scope of Work, Provision Four (4) Tasks has been revised to add the following: 4.15 Enter laboratory data into ADHS tracking databases; 4.16 Enter case data into ADHS eHARS registry; 4.17 Duplicate search; 4.18 Enter Out-of-State, RIDR, and CIDR call information; 4.19 Enter Partner Notification information; and 4.20 Phone calls, faxing, meeting preparation, follow up. 1.2. The Price Sheet is revised and replaced by the Price Sheet in this Amendment Three (3). All other provisions of this agreement remain unchanged. This grant award is reoccurring and has been awarded to the department in previous years. It is non-competitive and there is no cash or in-kind match required. The grant award is a mandated function; should it be discontinued, on-going contributions may be required. This grant deviates from County policy A2505 and does not allow for full indirect cost reimbursement, but a maximum of 15% of salaries and employee related expenses: Salaries and employee related expenses are estimated to be $201,131. The Department of Public Health’s indirect rate for FY20 is 19.2%. Total indirect expenses based on full award are estimated to be $38,305 of which $30,170 is recoverable and $8,135 Is not recoverable and will be subsidized by the MCDPH indirect cost pool. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. (C-86-13-003-3-15)

PURCHASE ORDER TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES HIV SURVEILLANCE PROGRAM Approve PO 293898 to Intergovernmental Agreement (IGA) Contract No. ADHS17-141746, HIV Surveillance program between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health. The PO reflects the quarterly amount of $28,952. Approval is requested for the annual amount of $115,808 for the CY21 budget period from January 1, 2021 through December 31, 2021. The term of the IGA is from January 1, 2017 through December 31, 2021. This grant award is reoccurring and has been awarded to the department in previous years.
