44. AMENDMENT TO LEASE AGREEMENT L-7441 WITH UMOM NEW DAY CENTERS, INC. FOR ON SITE PRIMARY CARE Approve and authorize the Chairman of the Board of Supervisors to execute Amendment No. 1, a renewal Lease Agreement No. L-7441 with UMOM New Day Centers, Inc. and Maricopa County by and through its Department of Public Health (MCDPH), Healthcare for the Homeless (HCH) for the County’s use of space (three exam rooms) at the UMOM Campus located at 3333 E. Van Buren, Phoenix, AZ for the purpose of providing primary care to the residents of the UMOM Campus and the homeless of Maricopa County. This Agreement is a no cost agreement. The term of this lease agreement commences on November 1, 2013 expiring on October 31, 2014. This Lease Agreement L-7441 was endorsed by the County’s Facility Review Committee during June of 2012. 49. NOTICE OF OPTION TO RENEW NO. 2 LEASE AGREEMENT WITH UMOM NEW DAY CENTERS, INC. FOR PRIMARY CARE SITE Approve and authorize the Chairman to execute Option to Renew No. 2 to Lease Agreement L-7441 with UMOM New Day Centers, Inc., a non-profit corporation as Licensor, for use of space at UMOM Campus located at 3333 E Van Buren, Phoenix, AZ for the purpose of providing primary care to the residents of UMOM Campus and the homeless of Maricopa County at no cost to the County. The term of the lease will begin November 1, 2014 and expire October 31, 2015. The term of the agreement is November 1, 2012 through October 31, 2017. The initial term of the agreement will terminate one (1) year from the date, with an option to renew the agreement four (4) additional one (1) year renewal terms. This lease provides space for three exam rooms, patient registration area, patient triage area, waiting room and bathroom. UMOM New Day Centers, Inc. is a non-profit corporation. There is no cost to presenting clientele. This is the second of four one year renewals. (C-86-13-006-1-02)

The term of the current license agreement will expire October 31, 2015. Amending the lease will allow the County to exercise the third of four one year renewal options. The term of the lease will commence November 1, 2015 and expire October 31, 2016. There is no fee to Maricopa County or to presenting clientele.
