53. AMENDMENT WITH ADHS FOR AZ NUTRITION NETWORK, SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM – EDUCATION AND LOCAL INCENTIVE AWARD SERVICES Approve Grant Amendment to Contract No. ( ADHS12-030680) between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to decrease funding for Arizona Nutrition Network (AzNN), Supplemental Nutrition Assistance Program – Education (SNAP-Ed) and Local Incentive Award (LIA) Services. The decrease amount reflect the changes due to the American Taxpayer Relief Act budget reduction. The revised Price Sheet is effective February 1, 2013, for the budget period October 1, 2012 through September 30, 2013. The decrease amount is for not-to-exceed $770,718 bringing the total from $3,082,874 to $2,312,156. The term is from October 1, 2012 through September 30, 2017. MCDPH’s indirect rate for FY 2012-13 is 15.8%. The full indirect costs estimated at $420,634 is reduced to $174,422, of which is fully recoverable. Indirect base is net of pass thru amount of $1,033,800. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. (C-86-13-016-G-01)

AMENDMENT TO GRANT WITH ADHS FOR AZ NUTRITION NETWORK, SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM – EDUCATION AND LOCAL INCENTIVE AWARD SERVICES Approve Amendment No. 2 for Grant Application (RFGA) No. ADHS12-00001365 authorized by Purchase Order No. ADHS12-030680 between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide funding for Arizona Nutrition Network (AzNN), Supplemental Nutrition Assistance Program – Education (SNAP-Ed) and Local Incentive Award (LIA) Services. This Amendment replaces the Price Sheet and extends the contract date through September 30, 2014. The funding amount is not-to-exceed $3,071,075 for the budget period October 1, 2013 through September 30, 2014. The term of this Agreement is from October 1, 2012 through September 30, 2017. Maricopa County’s indirect rate for FY2013-2014 is 15%. The full indirect costs are estimated at $262,123 which is fully recoverable. Payments to sub recipients in the amount of $1,061,468 are not subject to the indirect rate per OMB Circular A-87. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. AMENDMENT TO GRANT WITH ADHS FOR AZ NUTRITION NETWORK, SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM – EDUCATION AND LOCAL INCENTIVE AWARD SERVICES Approve Amendment No. 3 for Grant Application (RFGA) No. ADHS12-00001365 authorized by Purchase Order No. ADHS12-030680 between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide funding for Arizona Nutrition Network (AzNN), Supplemental Nutrition Assistance Program – Education (SNAP-Ed) and Local Incentive Award (LIA) Services. This Amendment pursuant to Terms and Conditions, Provision 2, Option to Renew Grant, the Grant is hereby extended through September 30, 2015. A subsequent Amendment will be issued which reflects changes to Scope of Work and Budget due to USDA requirements. The term of this Agreement is from October 1, 2012 through September 30, 2017. AMENDMENT TO GRANT WITH ARIZONA DEPARTMENT OF HEALTH SERVICES Approve a retro-active Amendment No. 4 to Grant Application (RFGA) No. ADHS12-00001365 authorized by Purchase Order No. ADHS12-030680 between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide funding for Arizona Nutrition Network (AzNN), Supplemental Nutrition Assistance Program – Education (SNAP-Ed) and Local Incentive Award (LIA) Services. This Amendment replaces the Price Sheet and the Scope of Work. The funding amount is in the not-to-exceed amount of $3,029,073 for the budget period October 1, 2014 through September 30, 2015. The term of this Agreement is from October 1, 2012 through September 30, 2017. All other terms and conditions will remain in full force and effect. This item is retro-active because we just received the amendment from ADHS. The Department of Public Health’s indirect rate for FY15 is 14.8%. The full indirect costs are estimated at $269,743 of which is fully recoverable. Indirect base is net of pass thru amount of $936,741, payments to sub-recipients in the amount of $936,741 are not subject to the indirect rate per OMB Circular A-87. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation.
