66. CONTRACT WITH CIRCLE THE CITY FOR CONTRUCTION OF TUBERCULOSIS ISOLATION ROOMS AND RELATED CARE SERVICES Approve Contract to a Public Health Request for Proposal 12-031 (PH RFP 12-031) between Circle the City (CTC) and Maricopa County through its Department of Public Health (MCDPH) for the construction and use of Tuberculosis Isolation Rooms and Related Care Services. The cost of construction for the project is estimated at $65.64 per square foot. The two isolation rooms total approximately, 366 square feet, thus the cost for construction of these rooms is estimated to be $24,024. Additional costs for the use of the rooms and related services are set at $190 per person/per day for year one. The term of this contract is from August 13, 2012 to August 12, 2017. PH RFP 12-031 contract was procured under Section MC1-330 of the Maricopa County Procurement Code, Competitive Sealed Proposals. This contractor was the successful respondent to a Request for Proposals issued by the MCDPH on August 13, 2012. Also, there is a request for the Board of Supervisors to approve PH RFP 12-031 contract. The item was endorsed by Facility Review Committee on September 27, 2012. (C-86-13-019-3-00)

39. AMENDMENT TO CONTRACT WITH CIRCLE THE CITY FOR TUBERCULOSIS RELATED CARE SERVICES Approve Amendment No. 1 to Contract to a Public Health Request for Proposal (PH RFP 12-031) between Circle the City (CTC) and Maricopa County through its Department of Public Health (MCDPH) for the use of Tuberculosis Isolation Rooms and Related Care Services. • The Contract term is extended to August 12, 2018 • Section 6.23 shall be deleted in its entirety • The Pricing Sheet is replaced with the Revised Pricing Sheet. The daily rate is increased to $274 per person/per day, for a not to exceed amount of $39,855 for the budget period August 13, 2017 through August 12, 2018, at which time this Contract expires. All other terms and conditions of this Contract remain in full force and effect. PH RFP 12-031 was procured under Section MC1-330 of the Maricopa County Procurement Code, Competitive Sealed Proposals. This contractor was the successful respondent to a Request for Proposal issued by the MCDPH on August 13, 2012. The BOS approved PH RFP 12-031 and the item was endorsed by Facility Review Committee on September 27, 2012. (C-86-13-019-3-01)
