62. CONTRACT WITH COMMUNITY BRIDGES, INC FOR THE PURCHASE OF BEHAVIORAL HEALTH SERVICES AHD PSYCHATRIC OVERSITE SERVICES TO HOMELESS PATIENTS Approve Contract to a Public Health Review of Qualifications 13-003 (PH RQQ 13-003) between Community Bridges, Inc, an Arizona corporations (CBI) and Maricopa County by and through its Department of Public (MCDPH), Healthcare for the Homeless for the purchase of behavioral Health Services and psychiatric over-site services to homeless patients. This contract will provide a reimbursement in an amount not-to-exceed $50,000. The contract term is one (1) year, beginning on the September 12, 2012 and ending September 11, 2013, with the option and with the agreement of the CBI, renew the term of this Contract for additional terms up to a maximum of five (5) years. PH ROQ 13-003 contract was procured under Section MC1-347 of the Maricopa County Procurement Code, Competitive Sealed Proposals. This contractor was the successful respondent to a Request for Proposals issued by the MCDPH on September 12, 2012. (C-86-13-020-3-00)

AMENDMENT TO CONTRACT WITH COMMUNITY BRIDGES, INC FOR THE PURCHASE OF BEHAVIORAL HEALTH SERVICES AHD PSYCHATRIC OVERSITE SERVICES TO HOMELESS PATIENTS Approve Amendment No. 1 to Contract to Public Health Review of Qualifications 13-003 (PH RQQ 13-003) between Community Bridges, Inc, an Arizona corporations (CBI) and Maricopa County by and through its Department of Public (MCDPH), Healthcare for the Homeless to remove and replace Exhibit A, Price/Compensation with a Revised Exhibit A, Price/Compensation. The Contract PH ROQ 13-003 provides behavioral Health Services and psychiatric over-site services to homeless patients. The contract term is one (1) year, beginning on the September 12, 2012 and ending September 11, 2013, with the option and with the agreement of the CBI, renew the term of this Contract for additional terms up to a maximum of five (5) years.
