MEMORANDUM OF UNDERSTANDING WITH PHOENIX ELEMENTARY SCHOOL DISTRICT FOR DENTAL SEALANT Approve a Memorandum of Understanding (MOU) between Phoenix Elementary School District and Maricopa County through its Department of Public Health to provide Dental Screenings and Dental Sealants to eligible schools and students within the Phoenix Elementary School District. This amendment extends the term from August 9, 2013 thru June 30, 2013 to July 1, 2013 thru June 30, 2014. All other terms and conditions of the original MOU shall remain in full force and effect. This amendment extends the term from July 1, 2013 thru June 30, 2014 to July 1, 2014 thru June 30, 2015. All other terms and conditions of the original MOU shall remain in full force and effect. This amendment extends the term from July 1, 2014 thru June 30, 2015 to July 1, 2015 thru June 30, 2016. Retroactive is due to processing time at school district. All other terms and conditions of the original MOU shall remain in full force and effect. (C-86-13-022-3-03)

AMENDMENT TO MEMORANDUM OF UNDERSTANDING WITH PHOENIX ELEMENTARY SCHOOL DISTRICT FOR DENTAL SEALANT PROGRAM SERVICES Approve a non-financial Amendment No. 5 to the Memorandum of Understanding (MOU) between the Phoenix Elementary School District and Maricopa County by and through its Department of Public Health, Office of Oral Health Dental Sealant Program for free dental sealant services for the students. The MOU is hereby amended as specified below: Background, Paragraph 4, Sentence 2 is amended to read: A licensed dental hygienist will provide written take-home findings and a list of dental clinics to all (insured and uninsured) second and sixth grade students who return “positive” parental consent forms. C. fourth bullet point is amended to read: Provide licensed dental hygienists and trained dental assistants to conduct the dental screening and dental sealant activities at each school.
