73. IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR FLUORIDE VARNISH SERVICES Approve the Intergovernmental Agreement (IGA) from the Arizona Department of Health Services (ADHS13-031613) and Maricopa County through its Department of Public Health, Community Health Action, Family Health Services, for the Fluoride Varnish Services to prevent tooth decay in children and adolescents. This grant award provides funding in the amount of $203,416 for budget period beginning September 1, 2012 through August 31, 2013. The grant term is September 1, 2012 through August 31, 2017. The Department of Public Health’s indirect rate for FY 2012-2013 is 15.8%. Full indirect costs are estimated at $29,218 of which $18,492 is allowable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funding for this grant will be provided by ADHS and will not affect the County general fund. (C-86-13-024-3-00)

40. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR FLUORIDE VARNISH SERVICES Approve Amendment No. 1 to the Intergovernmental Agreement (IGA) from the Arizona Department of Health Services (ADHS13-031613) and Maricopa County through its Department of Public Health for the Fluoride Varnish Services to prevent tooth decay in children and adolescents. Effective January 1, 2013, this Amendment replaces the revised Price Sheet, Page Twenty-Two (22) of the original Contract, with the revised Price Sheet, Page Three (3) of Amendment No. 1. The budget is reduced Not-to-Exceed $179,749 for budget period beginning September 1, 2012 through August 31, 2013. The grant term is September 1, 2012 through August 31, 2017. The revised Price Sheet includes the following line item changes: a. Personnel Services will decrease by $69,276 due to the County no longer hiring additional FTE’s for the program. b. ERE Services will decrease by $26,421 due to the reduction of the program FTE’s. c. Professional & Outside Services will increase by $80,640 due to the contracting of professional persons by Maricopa County to provide the program services instead of hiring additional FTE’s as originally purposed. d. Operating Expenses will decrease by $6,459 due to discrepancies between the submitted budget and the budget narrative. e. Indirect Expenses will decrease by $2,151 due to budget changes. The Department of Public Health’s indirect rate for FY13 is 15.8%. Full indirect costs are estimated at $25,818 of which $16,341 is allowable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. PURCHASE ORDER FOR IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR FLUORIDE VARNISH SERVICES Approve Purchase Order (PO) (ADHS13-031613:2) to the Intergovernmental Agreement (IGA) Contract No. (ADHS13-031613) between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) for the Fluoride Varnish Services to prevent tooth decay in children and adolescents. The PO is effective July 1, 2013 through June 30, 2014, for the budget amount not to exceed $179,749. The term of the contract is September 1, 2012 through August 31, 2017. Terms and conditions set forth in bids, quotation, or purchase order are incorporated herein by reference and become a part of this order. The Department of Public Health’s indirect rate for FY2013-2014 is 15%. This grant allows for only 10% indirect cost reimbursement. Full indirect costs are estimated at $24,511 of which $16,341 is allowable and $8,170 is unallowable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR FLUORIDE VARNISH SERVICES Approve Amendment No. 2 to the Intergovernmental Agreement (IGA) Contract No. (ADHS13-031613) from the Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide additional funding for the Fluoride Varnish Services to prevent tooth decay in children and adolescents. Item 1 will replace the price sheet effective August 1, 2014 through June 30, 2015, for an increased budget amount not to exceed $77,680 bringing the budget total amount from $179,749 to $257,429. The term of the contract is September 1, 2012 through August 31, 2017. This grant deviates from County Policy A2505 and does not allow for full indirect cost reimbursement. The Department of Public Health’s indirect rate for FY2014-15 is 14.8%. This grant allows for only 10% indirect cost reimbursement. Full indirect are estimated $10,451, of which $7,062 is allowable and $3,389 is unallowable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105.
