59. AMENDMENT TO CONTRACT WITH CITY OF TEMPE FOR ARIZONA NUTRITION NETWORK SERVICES, SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM – EDUCATION AND LOCAL INCENTIVE AWARD SERVICES Approve Amendment No. 1 (Contract authorized by Purchase Order No. ADHS12-030680) between the City of Tempe and Maricopa County by and through its Department of Public Health (MCDPH) to provide funding for Arizona Nutrition Network (AzNN), Supplemental Nutrition Assistance Program – Education (SNAP-Ed) and Local Incentive Award (LIA) Services. This Amendment extends the Contract date through September 30, 2014. The funding amount is not-to-exceed $485,178 for the budget period October 1, 2013 through September 30, 2014. The term of this Contract is from October 1, 2012 through September 30, 2017. All other terms and conditions remain in full force and effect. Maricopa County’s indirect rate for FY2013-2014 is 15%. This Amendment is a sub recipient and is not subject to indirect charges per OMB Circular A-87. This Contractor was a successful respondent to a Notice of Intent issued by the Maricopa County Department of Public Health pursuant to PH NOI 12-013. This Contract was procured under Section MC1-330 of the Maricopa County Procurement Code, Request for Proposal. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funding for this Agreement is provided by a Grant from ADHS and will not affect the County’s general fund. (C-86-13-031-3-01)

AMENDMENT TO SUB-RECIPIENT CONTRACT WITH THE CITY OF TEMPE Approve reto-active Amendment No. 2 to sub-recipient contract authorized by Purchase Order No. ADHS12-030680 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH) to provide funding for Arizona Nutrition Network (AzNN), Supplemental Nutrition Assistance Program-Education (SNAP-Ed) and Local Incentive Award (LIA) Services. This Amendment extends the Contract date through September 30, 2015. The funding amount is not-to-exceed $485,178 for the budget period October 1, 2014 through September 30, 2015. The term of the Contract is October 1, 2012 through September 30, 2017. All other terms and conditions remain in full force and effect. This item is retro-active due to schools being closed during the summer months. This Contractor was a successful respondent to a Notice of Intent issued by the Maricopa County Department of Public Health pursuant to PH NOI 12-013. This Contract was procured under Section MC1-330 of the Maricopa County Procurement Code, Request for Proposal. (C-86-13-031-3-02)

The term of the Contract is October 1, 2012 through September 30, 2017. The contract is amended as follows: A. 1.1, extend Contract period to October 1, 2015 through September 30, 2016. B. 6.27.1 Exhibit A, Budget; replace with an updated Exhibit A – Price Sheet/Fee Schedule in the not-to-exceed amount of $327,981.00. C. 6.27.2 Exhibit B, Scope of Work; replace with an updated Exhibit B – Scope of Work. All other terms and conditions of the original contract and its amendments shall remain in full force and effect.
