44. CONTRACT WITH CITY OF PHOENIX FOR ARIZONA NUTRITION NETWORK SERVICES Approve Contract for Public Health Notice of Intent 12-013 (PH NOI 12-013) between City of Phoenix and Maricopa County through its Department of Public Health (MCDPH), to provide Arizona Nutrition Network Services to the public. The contract period is October 1, 2012 through September 30, 2013, for the budget amount of $235,138. This contractor is a successful respondent to a Notice of Intent issued by the Maricopa County Department of Health pursuant to PH NOI 12-013 and procured under Section MC1-345 of the Maricopa County Department of Health pursuant to PH NOI 12-013. Also, this contract is funded by a grant from Arizona Department of Health Services (ADHS12-030680), Arizona Nutrition Network – Supplemental Nutrition Assistance Program – Education (SNAP-Ed) and does not affect the county general fund. 48. AMENDMENT TO CONTRACT WITH CITY OF PHOENIX FOR PROVIDING ARIZONA NUTRITION NETWORK SERVICES, SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM – EDUCATION AND LOCAL INCENTIVE AWARD SERVICES Approve Amendment No. 1 to Contract (authorized by Purchase Order No. ADHS12-030680) between the City of Phoenix and Maricopa County by and through its Department of Public Health (MCDPH) to provide funding for Arizona Nutrition Network (AzNN), Supplemental Nutrition Assistance Program – Education (SNAP-Ed) and Local Incentive Award (LIA) Services. This Amendment extends the Contract date through September 30, 2014. The funding amount is not-to-exceed $235,138 for the budget period October 1, 2013 through September 30, 2014. The term of this Contract is from October 1, 2012 through September 30, 2017. All other terms and conditions remain in full force and effect. This Contractor was a successful respondent to a Notice of Intent issued by the Maricopa County Department of Public Health pursuant to PH NOI 12-013. This Contract was procured under Section MC1-330 of the Maricopa County Procurement Code, Request for Proposal. (C-86-13-037-3-01)

AMENDMENT TO RFP CONTRACT WITH CITY OF PHOENIX FOR PROVIDING ARIZONA NUTRITION NETWORK SERVICES, SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM Approve retroactive Amendment No. 3 to Request for Proposal (RFP) Contract between City of Phoenix (COP) (137315) and Maricopa County by and through its Department of Public Health (MCDPH) to provide the Arizona Nutrition Network (AzNN), Supplemental Nutrition Assistance Program-Education (SNAP-Ed) and Local Implementation Services (LIS) program to participants in Maricopa County. The term of the Contract is October 1, 2012 through September 30, 2017. The amendment is retroactive due to the processing time with the contractor. The contract is amended as follows: A. 1.1, extend Contract period to October 1, 2015 through September 30, 2016. B. 6.27.1 Exhibit A, Budget; replace with an updated Exhibit A – Price Sheet/Fee Schedule in the not-to-exceed amount of $172,654. C. 6.27.2 Exhibit B, Scope of Work; replace with an updated Exhibit B – Scope of Work. All other terms and conditions of the original contract and its amendments shall remain in full force and effect. COP was a successful respondent to a Notice of Intent issued by the Maricopa County Department of Public Health pursuant to PH NOI 12-013.
