68. AMENDMENT TO AVONDALE ELEMENTARY SCHOOL DISTRICT FOR SCHOOL HEALTH IMPLEMENTATION PLAN Approve Amendment No. 1 Intergovernmental Agreement (IGA) between Avondale Elementary School District (AESD) and Maricopa County through its Department of Public Health (MCDPH), Office of Tobacco & Chronic Disease Prevention for the implementation of a School Health Implementation Plan which will provide AESD with the necessary curriculum materials and school supplies to complete the program. This Amendment increases funding by $1,000 for budget period July 1, 2012 through May 1, 2013, for a new aggregate not-to-exceed contract total of $9,000. This Amendment adds one additional school in Attachment A to the list of participating schools. This Amendment is effective upon execution by both parties. All other terms and conditions of the original Agreement that have not been amended by this or previous amendments shall remain in full force and effect. Funding for this agreement is provided by the Tobacco and Chronic Disease Grant from Arizona Department of Health Services (ADHS) and will not affect the County general fund budget. (C-86-13-041-3-01)
