65. IGA WITH WASHINGTON ELEMENTARY SCHOOL DISTRICT FOR SCHOOL HEALTH IMPLEMENTATION PLAN Approve an Intergovernmental Agreement (IGA) between Washington Elementary School District (WESD) and Maricopa County through its Department of Public Health (MCDPH), Office of Tobacco & Chronic Disease Prevention for the implementation of a School Health Implementation Plan (SHIP) which will provide WESD with the necessary curriculum materials and school supplies to complete the program. This is a one-time payment of $15,000 in which ten school within the district will receive $1,500 each with a final program report due at the end of project completion. The term of this Agreement is from October 1, 2012 through May 1, 2013. Funding for this agreement is provided by the Tobacco and Chronic Disease Grant from Arizona Department of Health Services and will not affect the County general fund budget. (C-86-13-047-3-00)

53. AMENDMENT TO IGA WITH WASHINGTON ELEMENTARY SCHOOL DISTRICT FOR SCHOOL HEALTH IMPLEMENTATION PLAN Approve Amendment No. 1 between Washington Elementary School District (WESD) and Maricopa County through its Department of Public Health (MCDPH), Office of Tobacco & Chronic Disease Prevention for the School Health Implementation Plan which will provide WESD with the necessary curriculum materials and school supplies to complete the program. This Amendment increases funding by $3,000 for budget term October 1, 2012 through May 1, 2013 for a new aggregate not-to-exceed contract total of $18,000. This Amendment adds two additional schools in Attachment A to the list of participating schools. This Amendment is effective upon execution by both parties. All other terms and conditions of the original Agreement that have not been amended by this or previous amendments shall remain in full force and effect. Funding for this Agreement is provided by the Tobacco and Chronic Disease Grant from Arizona Department of Health Services (ADHS) and will not affect the County general fund budget. (C-86-13-047-3-01)
