33. IGA WITH UNION ELEMENTARY SCHOOL DISTRICT TO PROVIDE FUNDING FOR A SCHOOL HEALTH INDEX PROGRAM Approve an Intergovernmental Agreement (IGA) between Union Elementary School District and Maricopa County through its Department of Public Health (MCDPH), Maricopa County Tobacco and Chronic Disease to provide funding for a School Health Assessment Program. The IGA budget amount is $2,000, for the budget period October 1, 2012 through May 1, 2013 with the option to extend for additional periods, not-to-exceed a total extended term of five years. Funding for this Agreement is provided by the Tobacco and Chronic Disease Grant from Arizona Department of Health Services and will not affect the County general fund budget. (C-86-13-059-3-00)
