CONTRACT WITH DELTA DENTAL PLAN OF ARIZONA FOR DENTAL INSURANCE SERVICES Approve Contract between the Delta Dental Plan of Arizona and Maricopa County through its Department of Public health for the provision of dental insurance services to individuals affected by HIV/AIDS. The term for this Contract is 03/01/2013 through 02/29/2016 and the Contract amount is not-to-exceed $3,103,912.00. Funding for this Contract is provided by a grant from the Federal Department of Health and Human Services, Health Resources and Services Administration (HRSA), the Ryan White (RW) Comprehensive HIV/AIDS Treatment Modernization Acts of 2006 and the Maricopa County General Government Department, Ryan White Part A Program and will not affect the County general fund. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Contract does not alter the budget constraining expenditures of the local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. (C-86-13-061-3-00)

AMENDMENT TO CONTRACT WITH DELTA DENTAL PLAN OF ARIZONA FOR DENTAL INSURANCE SERVICES Approve Amendment No. 1 between Delta Dental Plan of Arizona and Maricopa County through its Department of Public Health for the provision of dental insurance services to individuals affected by HIV/AIDS. This amendment revises section 1.6.11 - Contract Year-End Claims, 1.6.11.1 and 1.6.11.2 of the original contract. All other terms and conditions of the original contract shall remain in full force and effect. The Ryan White (RW) Comprehensive HIV/AIDS Treatment modernization Acts of 2006 and the Maricopa County General Government Department, Ryan White Part A Program provides dental insurance services to individuals affected by HIV/AIDS. AMENDMENT TO RFP CONTRACT WITH DELTA DENTAL PLAN OF ARIZONA FOR DENTAL INSURANCE SERVICES Approve Amendment No. 2 to Request for Proposal (RFP) Contract between Delta Dental Plan of Arizona and Maricopa County through its Department of Public Health for the provision of dental insurance services to individuals affected by HIV/AIDS. This amendment adds Section 1.3.5 to Section 1.3 Implementation and Ongoing Requirements of the base RFP contract. Section 1.3.5 delineates dental services outside of the country are not covered under the Delta Dental Plan. All other terms and conditions of the original contract and its amendments shall remain in full force and effect. Delta Dental Plan of Arizona is a successful recipient of Request for Proposal contract pursuant to PH RFP 13-006, dated March 1, 2013. AMENDMENT TO RFP CONTRACT WITH DELTA DENTAL PLAN OF ARIZONA FOR DENTAL INSURANCE SERVICES Approve Amendment No. 6 to Request for Proposal (RFP) Contract between Delta Dental Plan of Arizona and Maricopa County through its Department of Public Health to provide additional funding for the provision of dental insurance services to individuals affected by HIV/AIDS. This amendment will increase the contract amount by $2,000,000, bringing the not-to-exceed amount from $3,900,000 to $5,900,000, for the contract period March 1, 2016 to February 2019. Also, the contract amount is changed to an aggregate, not-to-exceed amount from $7,003,912 to $9,003,912 All other terms and conditions of the original contract and its amendments shall remain in full force and effect.
