34. IGA WITH PARADISE VALLEY UNIFIED SCHOOL DISTRICT FOR SCHOOL HEALTH IMPLEMENTATION PLAN Approve an Intergovernmental Agreement (IGA) between Paradise Valley Unified School District (PVUSD) and Maricopa County through its Department of Public Health (MCDPH), Office of Tobacco & Chronic Disease Prevention for the implementation of a School Health Implementation Plan which will provide PVUSD with the necessary curriculum materials and school supplies to complete the program. This is a one-time payment of $1,500 for each school listed totaling $1,500 with a final program report due at the end of project completion. The term of this Agreement is from October 1, 2012 through May 1, 2013. Funding for this agreement is provided by the Tobacco and Chronic Disease Grant from Arizona Department of Health Services and will not affect the County general fund budget. (C-86-13-063-3-00)
