48. FACILITY USE REVOCABLE LICENSE AGREEMENT WITH WESTAT, INC AT THE MARYVALE WIC SITE Approve full service Facilities Use/Revocable License Agreement (FURL) (P50221) between Westat, Inc., a Delaware corporation, Contractor , and Maricopa County, a political sub-division of the State of Arizona (County), by and through its Department of Public Health (MCDPH), Women, Infant and Children (WIC) Services to conduct a multi-state study for the United States Department of Agriculture, Food and Nutrition Services entitled Feeding My Baby, a National WIC Study. The study proposes to document and assess current infant and toddler feeding practices and dietary intake in eighty (80) WIC Sites. The Contractor has selected the MCDPH Maryvale WIC facility at 4002 N. 67th Ave, Ste 10, Phoenix AZ 85033. The term of this Agreement is for four months an shall commence July 1, 2013 to October 31, 2013. The Parties may extend the term by mutual agreement in writing. This FURL also allows for the Contractor use of clinic space at the Maryvale WIC Site as scheduled on Exhibit “A”. The Contractor agrees to provide the services at no cost to the County and the recipients. County agrees to provide access without monetary remuneration. The Facility Review Committee has reviewed and approved this FURL on September 27th, 2012. (C-86-13-069-M-00)
