36. CONTRACT WITH CHANDLER UNIFIED SCHOOL DISTRICT FOR IMMUNIZATIONS TO PROVIDE VACCINES TO ELIGIBLE CHILDREN Approve an Agreement between Maricopa County by and through the Department of Public Health (MCDPH) and Chandler Unified School District (CUSD) to provide vaccines to children who are eligible for the Vaccine for Children and to children who have private insurance with a plan that is contracted with MCDPH. The term of this Agreement becomes effective on the date both parties have signed and automatically renewals annually. Either party may terminate this Agreement at any time. (C-86-13-071-3-00)
