54. IGA WITH THE ARIZONA DEPARTMENT OF HEALTH SERVICES ELECTRONIC DISEASE SURVEILLANCE PROGRAM FOR SYNDROMIC SURVEILLANCE PROGRAM Approve an Intergovernmental Agreement (IGA) between the Arizona Department of Health Services (ADHS) Electronic Disease Surveillance Program (EDSP) (ADHS13-042810) and Maricopa County Department of Public Health (MCDPH) for the Syndromic Surveillance Program to perform outreach, recruitment and coordination of data reporting. The not-to-exceed amount is $39,400.00 for the budget period of March 1, 2013 through August 31, 2013, and the term of this agreement begins March 1, 2013 through February 28, 2018. The Department of Public Health's indirect rate for FY 2012-13 is 15.8%. Indirect Cost is estimated at $5,376 all of which is fully recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funding for this Agreement is provided by a Grant from ADHS and will not affect the County general fund. (C-86-13-075-3-00)

43. AMENDMENT TO IGA WITH THE ARIZONA DEPARTMENT OF HEALTH SERVICES FOR SYNDROMIC SURVEILLANCE PROGRAM Approve Amendment No. 1 to Intergovernmental Agreement (IGA) (ADHS13-042810) ( between the Arizona Department of Health Services (ADHS) and Maricopa County Department of Public Health for the Syndromic Surveillance Program to perform outreach, recruitment and coordination of data reporting. The not-to-exceed amount is $47,436 for the budget period of November 1, 2013 through August 31, 2014, and the term of this agreement begins March 1, 2013 through February 28, 2018. The Department of Public Health's indirect rate for FY 2013-14 is 15%. Indirect Cost is estimated at $6,187 all of which is fully recoverable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. (C-86-13-075-3-01)

46. ADMINISTRATIVE CORRECTION TO IGA AMENDMENT WITH THE ARIZONA DEPARTMENT OF HEALTH SERVICES FOR SYNDROMIC SURVEILLANCE PROGRAM Approve an Administrative Correction to the agenda item regarding Amendment No. 1 to Intergovernmental Agreement (IGA) between the Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health to provide funding for the Syndromic Surveillance Program originally approved on February 12, 2014. (ADHS Contract 13-042810, agenda item C-86-13-075-3-01) This Administrative Correction provides information regarding the amendment in addition to that previously provided, which updated the Price Sheet and replaces provision 6 (re: revised price sheet). The amendment also deletes provision 4.12 (scrutinized businesses) and provision 15 (re: fingerprinting/certification); revises the information in provision 18 (re: HIPAA requirements); and adds provision 20 (re: a DUNS identification number retirement).
