73. AFFILIATION AGREEMENT WITH VHS OF PHOENIX, INC. D/B/A PHOENIX BAPTIST HOSPITAL TO PROVIDE TRAINING EXPERIENCE FOR PHYSICIAN RESIDENTS IN CLINICAL SERVICES Approve the Affiliation Agreement entitled “Resident Rotation Training Agreement” between VHS of Phoenix, Inc. d/b/a Phoenix Baptist Hospital (PBH) and Maricopa County through its Department of Public Health (MCDPH) to provide public health training experience for PBH physician residents in MCDPH Clinical Services. TERMINATION OF VHS OF PHOENIX, INC DBA PHOENIX BAPTIST HOSPITAL RESIDENT ROTATION TRAINING AGREEMENT Approve the termination of Affiliation Agreement (AA) (C-86-13-099-3-00) entitled “Resident Rotation Training Agreement” between VHS of Phoenix, Inc. d/b/a Phoenix Baptist Hospital (PBH) and the Department of Public Health. The agreement is non-financial, and is effective from July 1, 2013 until June 30, 2018. Under item no. C-86-13-099-3-00, in reference to Section 5. Term and Termination. B., this AA has been replaced with a new Agreement (C-86-18-021-3-00), VHS of Phoenix, Inc., a Delaware corporation, dba Abrazo Central Campus (ACC), formerly known as PBH, thereby fulfilling notification to ACC and accommodating the residents participating at MCDPH.
