This Contract provides funding to SWHD in an amount not-to-exceed $447,958 for Contract period effective July 1, 2013 through June 30, 2014. The contract term is July 1, 2013 through June 30, 2018. AMENDMENT TO CONTRACT WITH SOUTHWEST HUMAN DEVELOPMENT FOR COMPREHENSIVE COMMUNITY HEALTH NURSING SERVICES Approve a Pre-designated Contract Amendment No. 1 between Southwest Human Development (SWHD) and Maricopa County through its Department of Public health, Office of Family Health and for comprehensive community health nursing services to eligible women and their family through the South Phoenix Nurse Family Partnership Program (SPNFP). This amendment extends the budget term from July 1, 2013 thru June 30, 2014 to September 30, 2014, with an increased contract amount for $120,000, bringing the not-to-exceed aggregate amount to $567,958. The contract term is July 1, 2013 through June 30, 2018. All other terms and conditions of the original contract shall remain in full force and effect. This Contract is funded through an Arizona Department Health Service grant, Nurse Family Partnership for MIECHV (ADHS12-023419) and does not affect the County general fund. (C-86-14-004-3-01)

The action item indicated an incorrect amendment number and paragraph 1.A. was previously approved in Amendment No. 2. Also, Approve pre-designated Contract Amendment No. 3 between Southwest Human Development (SWHD) and Maricopa County through its Department of Public health, Office of Family Health and for comprehensive community health nursing services to eligible women and their family through the South Phoenix Nurse Family Partnership Program (SPNFP). This subcontractor is exempt from the Maricopa County Procurement Code pursuant to Section MC1-102C, as they are a prior designated subcontractor. The not-to-exceed (NTE) aggregate budget amount is increased to $1,991,545.00 (increased by $615,000 for the extended budget term of October 1, 2015 through September 30, 2016). The attached budget is for a total of $750,000.00, $135,000.00 of which was approved as Amendment No 4 on 12/9/2015 under file number C-86-14-004-3-05. The original contract term is July 1, 2013 through June 30, 2018. Also, the retroactive is due to late issuance of the State Purchase Order. All other terms and conditions of the original contract shall remain in full force and effect.
